Restorative Nursing Program

We seek to maximize the quality of life for our long-term care population, who require a specialized restorative program. Of course, this service is included in the room and board fee. The therapy team and nursing staff develop individualized treatment plans for resident care in the areas of:
• Ambulation
• Range of motion
• Activities of daily living
• Positioning and alignment
• Skin integrity and wound management
• Contracture management
• Bowel and bladder training
• Rehabilitative dining
• Strengthening and restraint reduction
• Weekly and monthly weight monitoring
• Group Exercise Program
• Social Dining Program available

Dietary Program

• Provide meals to meet specific dietary and therapeutic needs
• Oral supplemental diets are available
• Regular nutritional assessments from our registered dietician

Activity Program

A positive and uplifting atmosphere is vital to the success of healing, recovery and rehabilitation. San Jose Healthcare & Wellness Center employs outgoing and friendly professional staff to create social programs and activities to fit the resident’s needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Friends and families are always welcome to visit and attend our special events.

Social Services

Our social service team assists both residents and their families during transitional periods. Social Service also provides Community Resource information. Will assist with Medi-Cal and Advance directives applications. Actively participate in meetings: care planning and discharges.

Rehabilitation Services

Our goal is to make a difference, every day, every time. Utilizing the best equipment in our newly remodeled rehab gym our highly educated and motivated staff work with you and your doctor to create the right rehabilitation plan.

Physical Therapy

Our licensed Physical Therapists focus on improving the coordination, balance, strength, endurance, and joint flexibility to achieve optimal levels of functional mobility. The goal is to move the resident toward greater independent mobility, either unassisted or with the aid of an assistive device. Rehabilitation education of the patients and families is an integral part of all treatment. For alleviating pain, preventing disability and restoring function following injury or illness, our Physical Therapists design individualized treatment programs that may include:
• Joint Mobilization
• Therapeutic Exercises
• Orthopedic Rehabilitation
• Wound Prevention
• Gait Transfer Training
• Range of Motion Exercises
• Modalities, including Ultrasound and E-Stim

Occupational Therapy

The Occupational Therapy program focuses on improving a person’s ability to manage everyday living skills. The Occupational Therapist evaluates each resident and identifies functional goals. Programs are individually developed to maximize the resident’s optimal level of independence, restoring functional abilities to compensate for dysfunction and minimize deficits. Occupational Therapy services may include:
• ADL Transfer/Mobility retraining
• Energy Conservation Techniques
• Upper Extremity Therapeutic Exercises
• ADL Retraining (Dressing, Grooming/Hygiene, Feeding, Bathing)
• Home Living Skills(Housekeeping,Cooking,Shopping,Financial
Management)
• Cognitive Perceptual Retraining
• Safety Instruction
• Utilization of Adaptive Equipment

Speech Therapy

Our licensed Speech-Language Pathologist focuses on restoring the patient’s ability to communicate. They address communication problems by helping patients improve skills in speaking, listening, thinking, reading, writing, problem solving as well as organizing and sequencing thoughts. Speech –Language Pathologist develops treatment and management programs and also assesses swallowing difficulties. This condition, if left untreated, often leads to aspiration of food into the lungs, Pneumonia, and other respiratory problems. Speech Therapy focuses on:
